New Dementia Care Planning Toolkit Released by Massachusetts Advisory Council on Alzheimer’s Disease and All other Dementias

The Care Planning Team of the Massachusetts Advisory Council on Alzheimer’s Disease and All other Dementias released a new toolkit for people living with dementia, dementia care partners, families, community-based service providers, and health care providers.

The purpose of the toolkit is to facilitate the development of person-centered dementia care plans that support living well with dementia and aims to help readers understand:

  • the importance and benefits of person-centered dementia care planning;
  • the unique characteristics of dementia that necessitate person-centered care plans; and
  • what dementia care planning involves and includes.

The toolkit also includes a list of other resources to connect Massachusetts residents to information for dementia care and support, encourage meaningful dementia care planning discussions, and promote equity and inclusion in dementia care planning.

There are two upcoming webinars to review the toolkit and its multiple resources and applications:

  1. Thursday, March 20 | 12:00 – 12:40 PM – This webinar is designed for people caring for family or friends living with dementia or those who live with dementia themselves.
  2. Thursday, March 27 | 12:00 – 12:40 PM – This webinar is designed for community-based service providers and health care providers.

The March 20th webinar registration is now available, as well as a flyer to help help spread the word. Please consider including the flyer announcement in a newsletter, listserv message, social media, or your website’s calendar of events. Additionally, organizations are encouraged to consider hosting a group viewing of either webinar.
