The organization Honoring Choices Massachusetts has announced the Who is Your Health Care Agent? consumer handout, which offers three simple steps to choose a Health Care Agent, complete a Health Care Proxy, and share your Proxy with your medical providers and others.

The free handout is available in 9 languages and links (and QR code) to the Honoring Choices Quick Start Page  to download a free MA Health Care Proxy, available in 15 languages.

Everyone can print the handout and distribute at medical offices, health clinics & fairs, and faith based and community events, schools- anywhere adults gather.
