Honoring Choices MA Partners & Friends are celebrating the 13th Annual Health Care Decisions Spring Celebration, in honor of April 16, which is National Healthcare Decisions Day. Community partners working to become more age- and dementia friendly may be interested in the organizations resources and toolkits.
All adults, 18 and older, can start to make their own personal care plan by downloading a free MA Health Care Proxy, a handy wallet card and Personal Directive (Living Will). Community groups and care professional organizations can co-host a fun and informative webinar with Honoring Choices. Just choose a date; we provide a virtual presentation with no cost tool kits.
