April is National Health Care Decisions Month. Every adult, 18 and older, can join the Honoring Choices MA 12th Annual Health Care Decisions Spring Celebration, April- June, to download a free Health Care Proxy to make a personal plan for good care today and over your lifetime.
It’s quick and easy to choose a trusted person to be your Health Care Agent and complete a free MA Health Care Proxy, available in 16 languages. If you get sick and are not able to make care decisions yourself, even for a short time while you get better, your Health Care Agent can step in with the power to get you the care you want.
